Sterile Processing Tech vs. Surgical Tech: What's the Difference?

Walk into any operating room and you'll see the work of two professionals who don’t get recognized as much as maybe they should: the surgical technologist who set up the room and the sterile processing technician who made sure every instrument in it was safe to use. While both careers are essential to safe surgery, the work itself is very different. One happens inside the operating room during procedures; the other happens behind the scenes, before and after. Here's how the two careers compare, and how to decide which one fits you.

What does a sterile processing technician do?

Controlling infections and keeping both surgeons and patients safe starts with this role. A sterile processing technician (also called a central service technician) ensures surgical instruments are safe to use. After a procedure, contaminated instruments come to the sterile processing department, where a sterile processing tech will decontaminate them, inspect them for damage or wear, package them into trays, and run them through sterilization equipment. They also keep records, manage inventory, and get sterile trays where they need to go for the next day's cases.

Most sterile processing techs work in a hospital's central sterile department, though they can also get hired by outpatient surgery centers, dental practices, and other facilities that perform medical procedures. The role is behind the scenes. Sterile processing technicians won't scrub in for operations or spend much time with patients, but every safe surgery in the building depends on their work.

What does a surgical technologist do?

A surgical technologist (often called a surgical tech or operating room tech) works inside the operating room as part of the surgical team. Before an operation, they perform the following job functions:

  • Prepare the room
  • Set up the sterile field
  • Lay out instruments
  • Help position the patient.

During surgery, surgical technologists pass instruments to the surgeon and keep count of supplies like sponges and needles. The U.S. Bureau of Labor Statistics reports that 71% of surgical technologists work in hospitals, with the remaining primarily working in outpatient care centers and physicians' offices.

It’s rewarding and important work to be sure, but it does ask a lot of you. You need to remain focused and stay on your feet for hours at a time. If sterile processing is precision behind the scenes, surgical technology is precision under pressure.

Key differences between sterile processing tech and surgical tech

The simplest way to think about it is that sterile processing techs make surgery safe before and after the operation, while surgical techs support the team during it.

  • Where you work: Sterile processing techs work in a dedicated department, usually away from patients. Surgical techs work in the operating room itself.
  • Patient contact: Minimal for sterile processing. Surgical techs are with patients through every procedure, even if those patients are under anesthesia for most of it.
  • Training time: Sterile processing has one of the shortest paths into healthcare, with certificate programs that can be completed in months. Surgical tech training requires an accredited program with supervised time in real operating rooms, which takes longer to complete.
  • Pace: Sterile processing is methodical and standards-driven. Surgical technology moves at the speed of the operation, and you can't step away mid-procedure.

Education and certification

You can train as a sterile processing technician through a certificate program in a year or less, depending on the program and time you’re able to commit to your studies. Next, you’ll need to earn the Certified Registered Central Service Technician (CRCST) credential from the Healthcare Sterile Processing Association (HSPA). Full certification requires 400 hours of hands-on experience, which can be paid or volunteer. You can complete those hours before the exam, or take the exam first and finish them within six months through HSPA's provisional certification option.

Surgical technology involves more schooling. To sit for the Certified Surgical Technologist (CST) exam from the National Board of Surgical Technology and Surgical Assisting (NBSTSA), you'll need to graduate from a surgical technology program accredited by CAAHEP or ABHES, unless you trained through a military program. Accredited programs include supervised clinical experience in the operating room, so unlike sterile processing, you can't complete the training entirely online. It can take anywhere from 12-24 months to complete training, depending on what type of program you pursue.

Salary and job outlook

According to the latest BLS wage data, surgical technologists earn a median hourly wage of $31.08 an hour, while medical equipment preparers, the BLS category that includes sterile processing techs, earn a median of $22.93 an hour.

Both fields are growing. The BLS projects employment of surgical assistants and technologists to grow 5% from 2024 to 2034, faster than the average for all occupations, with about 8,700 openings expected each year. Medical equipment preparers are projected to grow 10% over the same period, much faster than average. An aging population means more surgeries, and every one of them needs sterile instruments and a capable surgical team.

Which career is right for you?

Sterile processing might be your fit if you want a faster, lower-cost route into healthcare and you like methodical work with clear standards. It's also the better choice if you'd rather not be in the room during surgery. Surgical technology might suit you if you want to be part of the action in the operating room and you're willing to commit to a longer training program to get there.

There's also a practical middle path. Some people start in sterile processing to get hospital experience quickly, then use employer tuition programs to train for surgical technology later. The two departments work side by side every day, so experience in one is a real advantage in the other.
